We are seeking a dedicated and compassionate Home Healthcare Registered Nurse Case Manager to join our home healthcare team at JollyCare Residential Services, LLC. This role involves coordinating comprehensive care for clients with complex health needs, ensuring they receive appropriate caregiving services across various settings. The ideal candidate will have extensive nursing experience and possess a strong understanding of patient assessment, patient home healthcare plans, and utilization management. This position offers an opportunity to make a meaningful impact on client outcomes through personalized case management and collaborative care planning.
Duties
- Conduct thorough client assessments to determine their individual home health caregiving needs and develop personalized care plans.
- Coordinate with multidisciplinary teams to facilitate seamless transitions of care across inpatient, outpatient, and post-acute settings.
- Assure that inspections of clients’ home for environmental safety are completed and documented before a direct care worker is rostered.
- Monitor each client for a chronic medical condition during initial client assessment, during each home visit and/or immediately within 24 hours when a health concern has been reported by a direct care worker.
- Identifies patient care requirements by establishing personal rapport with patients and their families.
- Engage in utilization management by reviewing medical necessity and optimizing resource use.
- Document all patient interactions accurately in medical records while maintaining compliance with HIPAA regulations.
- Provide education on health management, medication adherence, and disease prevention strategies.
- Collaborate with providers in hospital medicine, emergency medicine, behavioral health, geriatrics, pediatrics, and primary care to coordinate comprehensive patient care.
- Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Assures quality of care by measuring health outcomes against patient care goals and hospital or regulatory standards and participates in quality improvement initiatives for clients and agency.
Requirements
- Valid RN license with current registration and compliance with state licensing requirements.
- Extensive nursing experience in home healthcare.
- Proven experience in post-acute care settings.
- Strong knowledge of physiology, medical terminology, diagnostic evaluation techniques, and discharge planning processes.
- Experience with patient assessment tools and vital signs monitoring.
- Familiarity with HIPAA regulations and medical records documentation standards.
- Skills in case management, utilization management, and discharge coordination within managed care frameworks.
- Ability to work collaboratively across disciplines including behavioral health and geriatrics populations.
- Excellent communication skills for patient service delivery and interdisciplinary team collaboration.
- Minimum of 1 year of recent home health nursing experience.
